You can only change your name in a current passport if the passport was issued less than one year ago. Otherwise, you will need to renew it, or apply for a new one.

Use form DS-5504 to make a change or correct a passport. Submit this form, along with your current passport and the original marriage license, divorce decree or court ordered name change. There is no fee if you submit your paperwork within a year of the passport issuance. After a year, you will need to apply for a new or renewal passport, and be charged the regular fees.
