What supporting documents will I need to renew my passport?

After completing, signing and dating the passport renewal application form, you will need to include the following supporting documents in your passport application package:

1. Your most recent U.S. passport book or passport card
2. A new passport photo
3. A completed and signed DS-82 passport application form
4. If your last name is now different than it was on the previous passport, you'll need a marriage certificate or court order showing the change

If you're renewing your passport with an expediter, you will also need to submit:

1. A printed travel itinerary
2. A Letter of Authorization, provided by your expediter
